The Kindred Hospital Baldwin Park has an inpatient wound care program dedicated in the management of existing wounds and in the prevention of new wounds.
The wound care team consists of experienced and caring physicians and nurses who specialize in the management and treatment of wounds. The team works together in order to promote the healing of existing wounds by providing the most optimal treatments available, in accordance with the etiology and severity of the wound. They will continue to strive and assist in the prevention of new wounds.

Your diet is ordered by your physician and depends upon your diagnosis and medical condition. Our staff of professional, registered dietitians is available to assess your nutritional requirements and provide individualized nutrition counseling.
Registered dietitians and diet technicians provide high-quality nutritional care to all patients and arrange for any special meals physicians have ordered for you. You may discuss your food preferences, food allergies and special dietary needs with a hospital dietitian, diet technician or your nurse.
Our in-room food service for patients features a spoken menu and personal attention from our dedicated staff. Limited selections of kosher meals are available for patients with restricted diets. Please ask your nurse for help in ordering these meals.
The Clinical Laboratory at Kindred Hospital Baldwin Park provides accurate and timely laboratory testing services for patients within the hospital and the community. The Laboratory strives to deliver services in the most clinically relevant and cost-effective manner by intelligently applying its sources of dedicated professional, leading edge technology and advanced Laboratory Information System.
The Clinical Laboratory is licensed by California State Department of Health Services, Accredited by Healthcare Facilities Accreditation Program, a program of the American Osteopathic Association and is certifies under the Department of Health Services for CLIA. The Laboratory participates in proficiency testing through the College of American Pathologists (CAP).
Laboratory adheres to the regulations and guidelines outlined by the State of California Business Professional Code, OSHA, AOA and other regulatory agencies. All employees are careful to honor patient rights, especially those dealing with confidential information.
The laboratory is staffed twenty four (24) hours a day, seven days per week for inpatients. The services provided on-site, includes: Hematology, Coagulation, Chemistry, Blood Bank, Microbiology, Urinalysis, Point of Care, and Pathology.
The Nursing Service Department is responsible for providing the patient and their families’ care that requires specialized knowledge, judgment, and skills derived from the principles of biological, physical, social, psychosocial and nursing process.
The practice at the hospital is based on the nursing process, professional practice models, and recognized standards of care and is consistent with all-applicable state and regulatory laws.
Patient care is provided by Registered Nurse (RN), Licensed Practical Nurse (LVN), Licensed Practical Technician (LPT), Certified Nursing Assistant (CNA) and other nursing support employees.
Patient care is accomplished through the execution of the nursing process, education for patient and family, fair and equitable management, quality improvement, and ongoing implementation and revision of nursing standards.
The Utilization Review Department consists of three distinct departments:
Although each department is separate, the staff must communicate with one another on a daily basis to help ensure effective patient care and successful discharge planning.
The Case Management department consist of RNs or LVNs with experience working in a transitional care hospital. Their responsibility is to review patient charts from the time a patient is admitted, throughout the hospital stay, and finally when he/she is discharged from the hospital. These reviews help to ensure that a patient continues to meet our admission criteria. The case managers are in frequent communication with physicians and the interdisciplinary team members.
Discharge planning includes the coordination of a patient’s discharge plan, whether the patient is going home, a Board & Care, skilled nursing facility, or subacute facility. The discharge planner has to communicate with patients, families, physicians, and the case managers to make sure that all parties involved are aware and agreeable to the discharge plan. The discharge planner also coordinates any other relevant issues, including home health, transportation, and other related matters.
The responsibility of Social Services often involves a multitude of tasks. Each patient that is admitted into Kindred Hospital is interviewed by the Social Worker. A psychosocial assessment is completed, and the patient’s family or other authorized representative. The social worker can meet with the patients or families if problems arise that require assistance. Each social worker has a minimum of a bachelor’s degree. The social workers coordinate with case managers in planning weekly patient care conferences which also includes physicians and an interdisciplinary team to ensure an effective and efficient care plan for the patients.
